You will receive no payment or other form of compensation for use of these items, and no identifying information (photos, addresses, phone numbers, contact information, last names, or uniquely identifiable names) will be used without your express advance permission.
You may specifically request that NO items be used for fundraising, marketing, or promotional purposes, but you must identify any requested restrictions in writing. We respect your right to privacy and assure you no identifying information or photos that you send to us will ever be publicly used without your direct consent.
We will keep your information for as long as it is necessary for the purposes set out in this notice, unless a longer retention period is required or permitted by law for business or healthcare-related purposes.
